Presenter:  Kim Hill Since 2000, Kim has been providing social work supports to families across the Waterloo Region. In 2018, Kim began a private practice to support individuals and caregivers who are managing a chronic health condition. Kim's practice provides support to families by assisting them in coordinating their care, navigating health care systems and providing advocacy to their care teams. The aim of this support is to improve health outcomes for both patients and their caregivers. Register
